All applicants requesting certified copies of Birth, Death or Marriage certificates by mail or fax must complete an application form and their signature must be notarized.

Request a Birth, Death, or Marriage Certificate

In order obtain a marriage license or ceremony, you must schedule an appointment. Call (707) 234-6823 to do so. We are open to the public, Monday - Friday, 8:00am - 5:00pm. Due to staff shortages, we only allow up to four groups of people at a time.

Certified copies of Birth, Death and Marriage certificates may be obtained from our office in four ways:

Online*:	Vital records available online at VitalCheck
In Person:	You may come into the office, complete a request, and pay the required fee. A copy will be made for you while you wait. Orders are made Monday through Friday - 8:00am until 4:45pm. Picture ID is required on all orders.
By Mail:	You may mail your application to our office along with the required fee. Your copy will be made within 24 hours of receipt and mailed back to you.
By Fax:	Requests for certified copies may be ordered by fax. For more information please call (707) 234-6823.
