The Child Health and Disability Prevention Program (CHDP) delivers health check-ups to infants, children and youth and provides care coordination to assist families with medical appointment scheduling, transportation, and access to diagnostic and treatment services
Children eligible for CHDP services are both:
- Children, birth through 18 years, from low to moderate-income families, and
- Medi-Cal eligible children, birth through 20 years of age
Health check-ups are needed on a regular basis throughout the child's life. Infants and toddlers need frequent health check-ups to keep them up-to-date on their immunizations. School age children need a check-up to enter kindergarten and every couple years; these exams may find health problems that could prevent a child from doing well in school. Teenagers and young adults need health check-ups too. They may have questions about their health that they can ask a health care provider. Health exams are often required when children play sports or go to summer camp and CHDP exams can be given every year to meet these requirements.
A CHDP exam should include:
- Growth and Developmental Check
- All Needed Shots
- Dental Screening and referral to the Dentist starting at 1 year
- Vision Screening
- Hearing Screening
- Nutrition Check-up and referral to WIC for children under 5 years
- Health Education
- Tobacco Education
- Test for Anemia, Blood Lead, Urine, TB and Others as Needed
Getting complete health check-ups for children is important because these exams may prevent serious health conditions and disabilities by finding medical, dental, developmental and/or mental health problems early. Referrals are made for any health or dental conditions identified during the exam. A CHDP program nurse provides follow-up and assistance to families for diagnosis and treatment of problems that are found.
CHDP check-ups are available from many CHDP enrolled health care providers in Mendocino County. They include:
- Family practice doctors
- Pediatricians
- Community Health Clinics
- Indian Health Clinics
The CHDP Program has a list of providers in all areas of the county.
